This is a basic 5th Edition Character sheet with the layout rearranged and all of the text replaced with the OpenDyslexic font to better assist those with dyslexia.

My son was recently diagnosed with dyslexia and he desparately wants to play D&D with his mother and me, so I made this with the layout relaid so that all of the primary necessities are along the left side.  As you go right, it gets more referential and less functional.  I also retyped everything in an assistive font so that it's easier for him to read.

I also took the time to rearrange the skills and saves under the stats while rearranging the stats into a row and the skills into columns so as to silo the information and make it easier to compartmentalize the data with his lower reading skill.  I added a "Stat + Proficiency" value at the top of each Skill Stack to act as an inline reference for skills/saves that you have proficiency with (those where you don't have proficiency, you just use the mod in the box above).

Design-wise, I tried to stick close to the official character sheets from WOTC so that his sheet won't look dramatically different from the other players at a casual glance.

I hope that this sheet helps others who have difficulty with reading and want to play.

Update:  I've added a Party Inventory Sheet as well as a second version of the Character sheet using a non-assistive font as some people liked the layout but didn't care for the font.

v2 Update:  Now includes a form-fillable PDF, some portions are auto-calculated for convenience.

I'm not dyslexic, but I love this layout. I was a huge fan of ability score-nested skills on the 4e Essentials sheets. It makes the game not only easier to teach, but easier to play.

Thanks! That's why I include a non-DA version in the pack of sheets as well. These sheets made it so much easier to teach new players which skills were associated with which stats.

I'm not dyslexic myself, but I absolutely love the alternate layout - having the various proficiencies grouped with their respective skills is so useful!

Quick question - would you consider bringing out an alternative version for Excel or other similar spreadsheet apps in the future? I mainly use Excel or iWork Numbers for my character sheets for ease of changing details!

Thanks, I'm very proud of the layout, I used some of my UX training to try and make it easier to follow the data logically and with the intention that like data would be grouped for easier reference for those who have more difficulty reading it, and it ended up being just a really nice overall layout. I use it for all of my gameplay now too.

I wasn't really considering doing an Excel or iWork Numbers version, as there hasn't really been a high demand for them. In my own game, we play on paper copies. I don't have a lot of free time these days, but I'll look into how difficult it'll be to convert this for one of those for you.

Looks like I never got to it, if you need one, I can assemble one but I find that the spellcasting sheet provided by Wizards is usually sufficient as there is little written content on it.

Loved the design. Nearly half my gaming group is dyslexic to varying degrees and the font does somehow magically make it easier. It's a bit of a shame there is no spell-casting sheet but thank you for all your work :)

Is there any way to get the text boxes to scale text to fit? One of my players wanted to use this, but they ran into that issue

If you hit CTRL-E while filling out a field you can get a format editing option, you can change font size from the pop-up mini-window and if you need to change the line-height to fit more lines, you can hit the "More" button and then go over to the "Paragraph" tab and set a negative value on the "above" and/or "below" entries to space the text as you need.
The lines in the boxes won't line up with the text, so if you'd like me to add versions without the background lines, I can add those. Just let me know.

The latest version should be easier to print, I flattened most of the layers and fully embedded the font so it's smaller and should be more print-friendly.

These products were created by scanning an original printed edition. Most older books are in scanned image format because original digital layout files never existed or were no longer available from the publisher.

For PDF download editions, each page has been run through Optical Character Recognition (OCR) software to attempt to decipher the printed text. The result of this OCR process is placed invisibly behind the picture of each scanned page, to allow for text searching. However, any text in a given book set on a graphical background or in handwritten fonts would most likely not be picked up by the OCR software, and is therefore not searchable. Also, a few larger books may be resampled to fit into the system, and may not have this searchable text background.

For printed books, we have performed high-resolution scans of an original hardcopy of the book. We essentially digitally re-master the book. Unfortunately, the resulting quality of these books is not as high. It's the problem of making a copy of a copy. The text is fine for reading, but illustration work starts to run dark, pixellating and/or losing shades of grey. Moiré patterns may develop in photos. We mark clearly which print titles come from scanned image books so that you can make an informed purchase decision about the quality of what you will receive.
Original electronic format
These ebooks were created from the original electronic layout files, and therefore are fully text searchable. Also, their file size tends to be smaller than scanned image books. Most newer books are in the original electronic format. Both download and print editions of such books should be high quality.
