Shwaffle's 5e Encounter Sheet

This sheet is intended to make a DM's prep time significantly shorter by compiling all the necessary info needed for a combat encounter in seconds. DM's are able to modify most data after it has populated onto the sheet allowing for any customizing you would like!

I will always have this available for free because I want to help all DM's with their campaign planning. I do this in my free time as a coding hobby. If you feel this tool is worth it, please give it a review and share with your friends! If you love it feel free to throw me some beer money, but you are not obligated to.


  • Manuals to add: Mines of Phandelver; Monster a Day; Monster Hunter for 5e
  • Create 'lists' like Monster types, terrain, plane etc
  • Add option for the sheet to roll initiative for selected monsters (Will not be adding options for Player characters)
  • Add encounter Challenge Rating based on the monsters selected (Encounter Difficulty)
  • Optimize Attacks and Actions to remove repetative text and make lines overall less wordy
  • Ability to import custom data


v1.63 (12/27/18)

  • Small update, added Lost Mines of Phandelver to the sheet.
  • Hid the encounter difficulty part for the time being, as I have been getting negative reviews and feedback due to it.

  • Fixed issue with 'Skills' not showing properly
  • Added in the Constitution Modifier for the hit dice due to demand

v1.61 (7/19/18)

This one is all bug fixes! I highly recommend deleting all previous copies to make sure you're using this version. I would also love to know if this alerts people when I update? Is there any other way I should be alerting people? Let me know.

  • Went through and tested all monsters so none of them should error out. Please report any oddities and bugs on GitHub
  • Changed the verbiage of two-handed variant attacks in the descriptions to make it more apparent
  • Fixed a bug where some attacks wouldn't show based on the ability
  • Small code changes for consistency

v1.6 (7/7/18)

This feels like a small update due to the changelog, but I think the way I reworked how you choose a monster is such a big addition that I wanted to push it out as quickly as possible. This has been something I've wanted to add for quite a while so it feels nice that it's finally finished. This is the groundwork I needed to get the different lists into the sheet so let me know what you think!

Per usual I took too long with me recent update. This is a big one so I hope it makes up for the delay in the new manuals! I would love to have some input from users on what they think needs to be changed or added. After submitting this file, I'm going to begin work in a single page monster sheet. This will be extremely beneficial for boss level monsters that have a large traits/actions section. After that, I would like to work on an initiative order page! That sounds like a fun challange so I hope it turns out well. I appreciate everyones support and thank you for using my sheet!   • Fixed the font size of Speed so it's actually readable.
  • Encounter XP was only pulling the XP from the top monster. I am very sorry for those that relied on this and were given the wrong information.
  • Previous versions of the sheet would disable form highlighting for Adobe globally. This has been fixed to re-enable form highlighting on close.
  • Fixed a lot of errors in Monster Imports


  • Added Tome of Beasts.
  • Added Mordenkainen's Tome of Foes.
  • Added The Tortle Package.
  • Added radio buttons at the top to select whether or not you would like Adobe's field highlighting. I think it looks better off, but choose what you'd like!
  • Added some nice icons for the manuals you select.

He not only pointed me in a great direction to get this completed, he wrote up small section of code to showcase what his sheet does in a simple manner. I'm sorry if I'm a total noob, but I've used this form once and loved it, but now that I'm trying to use it again I can't get the monster list to show up. I click on "Choose Monster" after selecting my book, but all it does is clear any information in the monster areas without pulling up a list or any way for me to select a monster type.

What pdf viewer are you using? Make sure you only open it in Adobe Reader. Try downloading a new file and test that, if it happens again let me know

I feel like I have been giving some false promises lately that I haven't been able to keep. (mostly to those that have asked me questions in email)

Mad Mage/Dragon Heist are on my todo, but I am also working on v2 of the sheet which has a lot of QOL updates on the back end for me, but also frees up some real estate on the pages and makes things easier for additions in the future. I really hope everyone likes it, considering I've been teasing it for ages.

Transcribing the monster data is extremely time consuming and I have been working on things as I can. I thank everyone for their continued patience as I balance everything the best I can.

Hi all, would anyone have any advice as to how to get this to work in Linux?
I can read the PDF with Master PDF Editor 5, but I can't seem to auto-fill out or even choose any monsters, it's just blank fields.
To be clear, nothing happens when I click on the "Choose Monster" button.

Hi! Sorry for my late reply, I don't get notifications for messages here.

With the Javascript used within the PDF, Adobe Reader is required. I did some searching and I do not see a Linux compatible reader that supports Javascript. You could try using Wine, but I do not know how if it will work the same as on a standard Windows or Mac operating system.

Wishlist - no grey boxes (or at least, optional grey boxes) to save on toner when I print them out.

As a new DM this has changed my DMing world. The time I save now is crazy. Thank you so much for maintaining this!!

I am having one issue and I am most likely doing something wrong, but when I fill the radio button on the HP block, it adds XP to the XP box at the top, but it seems to be pulling only from "Monster 1's" XP stat.

Monster 1 = Gelatinous Cube = 450xp
Monster 2 = Gray Ooze = 100xp
When I select 1 Cube and 2 Oozes, XP = 1350

I swapped them to test:
Monster 1 = Gray Ooze = 100xp
Monster 2 = Gelatinous Cube = 450xp
When I select any 3, XP = 300

Thank you for letting me know and thank you for the review! This will be fixed on the next version. I should be able to find time this week to submit my next update.

This was an unfortunate oversight. Looks like it's been there since I made the function for that and had hardcoded the info to test. Silly me.

As many others have said, this is a great tool. I also appreciate you keeping up work on it. I am not to familiar with creations of this sort, so there could be an easy thing like a "Duplicate Page" option, but any chance there could be a version that contains 4 or 5 (or more) different variety of monsters Monsters(e.g. a Kobold, Winged Kobold, Kobold sorcerer, Kobold chieftan).

There isn't a "Duplicate Page" option, but if you need more than two monsters in a fight you would just need to select two, print that page, then select others and print those. The intention of my sheet is to display all technical combat data, not just basic info. I originally tried to have three monsters on a sheet, but there just isn't enough room for all the information.

I have made plans to make another sheet with just a single monster on it, however, to enable DM's to have a sheet for a major boss, and other legendary monsters that have a significant amount of information needed for their attacks.

Is it possible to add additional pages for extra monsters, but without the initiative boxes? So that one shhet could be the primary, and the rest working as a monster reference?

I love this sheet, arguably one of the coolest purchases I've made on the DM's Guild. I'll be trying it out for the first time in my game tonight! However, I did have a couple of questions.

First off, it seems "Monster 2" has more limited options than "Monster 1," seemingly only boss-type or named monsters. Is this intentional, and could it be changed to include generic monsters? I tend to run multiple different minions, and it's mildly inconvenient to need multiple sheets with a lot of unused space to do so.

Second, what does the button under the monster name do? When I check it, the monster name disappears and star appears in the box, but I can't identify any other changes.

Thirdly, do you have plans to support more than 2 monsters in a sheet? I'm personally picturing a fill-in box near the top for "number of different monsters," which would then create additional blank stat blocks for each monster that operate as they currently do. I think...See more

Thanks for loving the sheet! Sorry for the delay in response, it seems that DMsGuild doesn't email me anymore when I get posts. I need to see why that stopped.

1. Both sections are identical. You need to choose the manual you would like it to use at the top of the sheet(, as each section can use a different source (Like using Storm King's Thunder for one, and Monster Manual for the bottom). When I post, I do select manuals and a monster for each to show new users what fills out. Looks like I posted it with Yawning Portal for the top, and Tyranny of Dragons for the bottom. Select a different manual and you'll see some changes.

2. All forms have popup windows that explain what they do, so make sure to hover over an item and it will tell you about it. That particular radio button allows you to choose a monster, select the radio and then make a custom name for it. First, I love the sheet; great work! I have a very minor suggestion for an improvement if I may?
I know the sheet is fairly full, but would it be possible to add a field for an encounter name / number / reference? It would be handy to have an easy way to reference an encounter page from another document.
I was thinking perhaps in the top box, if you aligned the encounter and party fields to the bottom, there would be room for a text field above them for a name.

I would have to agree with Daniel. Having a name for the encounter would be helpful. It would help as I have to write on top of the sheet of what encounter it is. Most encounters aren't too bad. Buti just printed out a whole list for the Storm Kings Thunder Random encounters and have to write on the top of each.

Sorry for not responding sooner! DMsGuild did not notify me that anyone posted. I like this idea, I had the notes there somewhat for this purpose, but I am redoing the top part of the sheet a bit, and have been wondering what to fill all that open space with! Thanks for the suggestion. I'm about a week away from pushing out another update, so I think I'm going to get this in with it.

Hey Justin, My upcoming update will have all modules except for Tyranny of Dragons, and Tomb of Annihilation. I plan to have Tome of Beasts, ToA, and the Tyranny modules completed before Mordenkainen's Tome of Foes comes out!

Hey Richard,

Are you meaning variants like Deva, Planetar, & Solar (all angels)? If yes, then I am working on getting ALL monsters added to the sheet, it just takes some time.

If you mean creating custom monsters, that is towards the end of my todo list currently because I want to get all of the published monsters included first. After that, I will have to do some decent modification to the sheet to enable people to fully customize their monster.

I'm referring to leader types, humanoids with classes, etc. I've actually been able to make the current version work because I can modify everything on the main sheet except the name. The only other thing I can think of would be to have the MOD and SAVE change automatically when you give your monster different stats.

So I've been thinking about this, and I think I'm going to make a separate sheet where people can manually input stats, and it will change the info accordingly. Possibly have a list of attacks etc. It's on my todo list, I'll keep people updated!

Thanks for the compliment, I really appreciate it. Below is what is currently in the sheet. Originally I only had info from the SRD, but have since gotten all the data for the full Monster Manual, and separated out what was missing.

This encounter sheet currently has ~325 monsters from the SRD and 135 from Volo's. I am currently working on adding another ~125 creatures from the Monster Manual as well as monsters specifically designed for the different stories.

These products were created by scanning an original printed edition. Most older books are in scanned image format because original digital layout files never existed or were no longer available from the publisher.

For PDF download editions, each page has been run through Optical Character Recognition (OCR) software to attempt to decipher the printed text. The result of this OCR process is placed invisibly behind the picture of each scanned page, to allow for text searching. However, any text in a given book set on a graphical background or in handwritten fonts would most likely not be picked up by the OCR software, and is therefore not searchable. Also, a few larger books may be resampled to fit into the system, and may not have this searchable text background.

For printed books, we have performed high-resolution scans of an original hardcopy of the book. We essentially digitally re-master the book. Unfortunately, the resulting quality of these books is not as high. It's the problem of making a copy of a copy. The text is fine for reading, but illustration work starts to run dark, pixellating and/or losing shades of grey. Moiré patterns may develop in photos. We mark clearly which print titles come from scanned image books so that you can make an informed purchase decision about the quality of what you will receive.
Original electronic format
These ebooks were created from the original electronic layout files, and therefore are fully text searchable. Also, their file size tends to be smaller than scanned image books. Most newer books are in the original electronic format. Both download and print editions of such books should be high quality.
