Shwaffle's 5e Encounter Sheet

Adobe Acrobat XI or above is supported.

This sheet is intended to make a DM's prep time significantly shorter by compiling all the necessary info needed for a combat encounter in seconds. DM's are able to modify all data after it has populated onto the sheet allowing for any customizing you would like!

I will always have this available for free, because I ultimately want to help all DM's with their campaign planning. I do this in my free time as a coding hobby. If you feel this tool is worth it, please give it a review and share with your friends! If you love it, feel free to throw me some beer money, but you are not obligated to.

  • Add option for the sheet to roll initiative for selected monsters (Will not be adding options for Player characters)

v1.4 (2/18/18)


  • I really, really broke actions in 1.32 somehow. This has been fixed, and I can't believe I hadn't noticed earlier.
  • Fixed multiple Spellcasting blocks
  • STR mod had a 2 character limit, which broke when monsters had anything over +9 to the mod. This has been fixed.
  • Fixed a lot of data issues that weren't properly coded.


  • Redid the manual selections at the top, this will allow for a lot more adaptability.
  • Manually changing the ability score now modifies saves and mods.
  • Added a bunch of tooltips!
  • Added Out of the Elemental Evil (Out of the Abyss), Tyranny of Dragons (Hoard of the Dragon Queen, Rise of Tiamat), Curse of Strahd, Storm Kings Thunder, and Yawning Portal.
  • Added a lot of missing monsters, thank you to those who have let me know!
  • Added encounter name and location at the top, suggested by users!
  • Underneath the monster's name you can now select a checkbox to manually change the name of the monster without it killing the stats. Suggested by users!
  • Added Type & Subtype if applicable!
  • You can now search for a monster name! 
    • It is case sensitive
    • Adobe DC currently has a bug that makes this feature not 100% reliable. However, being able to search in the dropdown should help a significant amount with the major manuals.
  • Going to be working hard to get Tome of Beasts complete before Mordenkainen's Tome of Foes is released in May.

v1.32 (10/20/17)

  • Split some variants into their own monsters
  • Organized spells so they display nicer
  • Trying new Clear Form buttons
  • Traits and Actions will begin to display nicer when they have multiple paragraphs
  • Skills added
  • Rest of Monster Manual added
  • Began work on allowing DMs to have initiative rolled for the monsters. Will have both singular initiative and group initiative. (The extra radio button that pops up when you choose the monster won't do anything for now)
  • I was doing a lot of little changes and may have forgotten to patch some holes. If you find anything that breaks the sheet let me know!

v1.31 (7/3/17)

  • Checking the radio boxes near the HP boxes will now add monster XP to the total encounter experience
  • Passive Perception and Proficiency Bonus now have their own boxes
  • Immunities and Resistances have been moved to "Creature Info"
  • Armor Type has been added, will display a shield when the creature has one wielded
  • Top of the encounter sheet has been redone. Unavailable manuals are greyed out
  • Made the 'Clear' buttons a little better. I don't like the 'Clear Form' one, I'm thinking of what I want to change that to
  • Fixed some traits not populating correctly from Volo's
  • Began work on encounter difficulty

I love this sheet, arguably one of the coolest purchases I've made on the DM's Guild. I'll be trying it out for the first time in my game tonight! However, I did have a couple of questions.

First off, it seems "Monster 2" has more limited options than "Monster 1," seemingly only boss-type or named monsters. Is this intentional, and could it be changed to include generic monsters? I tend to run multiple different minions, and it's mildly inconvenient to need multiple sheets with a lot of unused space to do so.

Second, what does the button under the monster name do? When I check it, the monster name disappears and star appears in the box, but I can't identify any other changes.

Thirdly, do you have plans to support more than 2 monsters in a sheet? I'm personally picturing a fill-in box near the top for "number of different monsters," which would then create additional blank stat blocks for each monster that operate as they currently do. I think...See more

First, I love the sheet; great work! I have a very minor suggestion for an improvement if I may?
I know the sheet is fairly full, but would it be possible to add a field for an encounter name / number / reference? It would be handy to have an easy way to reference an encounter page from another document.
I was thinking perhaps in the top box, if you aligned the encounter and party fields to the bottom, there would be room for a text field above them for a name.

I would have to agree with Daniel. Having a name for the encounter would be helpful. It would help as I have to write on top of the sheet of what encounter it is. Most encounters aren't too bad. Buti just printed out a whole list for the Storm Kings Thunder Random encounters and have to write on the top of each.
Love the sheet. Hope to see the planned update.

Sorry for not responding sooner! DMsGuild did not notify me that anyone posted. I like this idea, I had the notes there somewhat for this purpose, but I am redoing the top part of the sheet a bit, and have been wondering what to fill all that open space with! Thanks for the suggestion. I'm about a week away from pushing out another update, so I think I'm going to get this in with it.

Hey Justin, My upcoming update will have all modules except for Tyranny of Dragons, and Tomb of Annihilation. I plan to have Tome of Beasts, ToA, and the Tyranny modules completed before Mordenkainen's Tome of Foes comes out!

Hey Richard,

Are you meaning variants like Deva, Planetar, & Solar (all angels)? If yes, then I am working on getting ALL monsters added to the sheet, it just takes some time.

If you mean creating custom monsters, that is towards the end of my todo list currently because I want to get all of the published monsters included first. After that, I will have to do some decent modification to the sheet to enable people to fully customize their monster.

I'm referring to leader types, humanoids with classes, etc. I've actually been able to make the current version work because I can modify everything on the main sheet except the name. The only other thing I can think of would be to have the MOD and SAVE change automatically when you give your monster different stats.

So I've been thinking about this, and I think I'm going to make a separate sheet where people can manually input stats, and it will change the info accordingly. Possibly have a list of attacks etc. It's on my todo list, I'll keep people updated!

Hey Richard, the update I'll be putting out in the next week allows you to change the name and modify the abilities, which will change the MOD and SAVE's.

Thanks for the compliment, I really appreciate it. Below is what is currently in the sheet. Originally I only had info from the SRD, but have since gotten all the data for the full Monster Manual, and separated out what was missing.

This encounter sheet currently has ~325 monsters from the SRD and 135 from Volo's. I am currently working on adding another ~125 creatures from the Monster Manual as well as monsters specifically designed for the different stories.

maybe I'm missing something but when I open the file it is pre filled I was under the impression that it was something that I could edit myself can somebody help me?

What version of Acrobat are you using? When you open the file, you should be able to see a drop down menu button where the monsters name is. Here is the button I'm describing:

Just a quick heads up, although this file is awesome I've noticed the Zombie auto-fill is creating Super Smart, but weak zombies by switching the CON and INT values found in the MM.

This has been a splendid addition to my library of resources! One question though, is will you be adding further monsters later on? I have noticed certain monsters are missing here and there (Fire Snakes, Troglodytes).

I have a request, but not for this product (it's pretty great as is)
Do you think you could do a form fillable PDF sheet for locations? I am desperately in need of a crib sheet for listing multiple NPCs encountered in one area. I'm thinking portrait orientation with 8 form fillable spaces for NPC info along the top then three larger notes areas for the encounter area details, plot quest details, and side quest details.
This would be perfect for helping me summarize social interactions and remember key points in a location.

These products were created by scanning an original printed edition. Most older books are in scanned image format because original digital layout files never existed or were no longer available from the publisher.

For PDF download editions, each page has been run through Optical Character Recognition (OCR) software to attempt to decipher the printed text. The result of this OCR process is placed invisibly behind the picture of each scanned page, to allow for text searching. However, any text in a given book set on a graphical background or in handwritten fonts would most likely not be picked up by the OCR software, and is therefore not searchable. Also, a few larger books may be resampled to fit into the system, and may not have this searchable text background.

For printed books, we have performed high-resolution scans of an original hardcopy of the book. We essentially digitally re-master the book. Unfortunately, the resulting quality of these books is not as high. It's the problem of making a copy of a copy. The text is fine for reading, but illustration work starts to run dark, pixellating and/or losing shades of grey. Moiré patterns may develop in photos. We mark clearly which print titles come from scanned image books so that you can make an informed purchase decision about the quality of what you will receive.
Original electronic format
These ebooks were created from the original electronic layout files, and therefore are fully text searchable. Also, their file size tends to be smaller than scanned image books. Most newer books are in the original electronic format. Both download and print editions of such books should be high quality.
