New question: Flesh rot description says "At the end of each short or long rest, the infected creature must succeed on a Constitution saving throw or gain 1 level of exhaustion as their body begins to decay." In the paragraph prior to that it says "After 1d4 days the creature gains 1 level of exhaustion..." My question is, does the exhaustion start taking effect after the character's FIRST short/long rest or short/long rests beginning in 1d4 days?

Good question.
The intent is that symptoms appear after 1d4 days with the first level of exhaustion (prior to which the disease is incubating) after which the character begins to make saving throws.

I bought this and was not very happy with it.
The diseases don't follow the format in the DMG...such as having immediate/tertiary effects using conditions. They are a bit wordy and some contain unnecessary information that wastes time to read in the middle of a game.
Regarding poisons I stopped immediately after the first one 'cyanide'
It was apparent that no research was done to translate a real world poison into the game. The effect saying 'you can't eat or drink' makes no sense at all.
Cyanide easily translates to direct necrotic damage or suffocation.
TLDR: Good for ideas but not refined or tested enough to even be usable. Seems like a first draft as of my reading on 5/13/16

I david, i really like your work and i bought a couple of them. They are well made but it would be awesome if you could make a printer friendly version of all of them. (I hate to read on a screen and i print everything) Thank you very much and keep up the good work, we like it!

Nice work, but it lacks something: A table with the effects in a quickly accessible form. This would be very helpful, better than just scanning the text or writing it out yourself.

Should "black vomit" cause exhaustion? I'm guessing since they can't keep food and water down that's the effect. Also is Bestial Rage called "crimson fury" too? It references that in the description. With Bestial Rage would a caster maybe go nuts and start blasting cantrips around instead of melee attacks?

Black vomit: If you don't eat, you become exhausted, so that's handled via the starvation rules.

Bestial rage: This changed names numerous times during writing. Crimson fury was an old name. I'll have to do a revision that either removes that reference or explicitly gives it as an alternate name. Thanks!

I went back and forth over whether an enraged caster would use cantrips or not. I pictured a sorcerer using cantrips since its an inherent ability but a wizard seemed less likely. But clarifying the distinction in the text was wordy and potentially confusing.

These products were created by scanning an original printed edition. Most older books are in scanned image format because original digital layout files never existed or were no longer available from the publisher.

For PDF download editions, each page has been run through Optical Character Recognition (OCR) software to attempt to decipher the printed text. The result of this OCR process is placed invisibly behind the picture of each scanned page, to allow for text searching. However, any text in a given book set on a graphical background or in handwritten fonts would most likely not be picked up by the OCR software, and is therefore not searchable. Also, a few larger books may be resampled to fit into the system, and may not have this searchable text background.

For printed books, we have performed high-resolution scans of an original hardcopy of the book. We essentially digitally re-master the book. Unfortunately, the resulting quality of these books is not as high. It's the problem of making a copy of a copy. The text is fine for reading, but illustration work starts to run dark, pixellating and/or losing shades of grey. Moiré patterns may develop in photos. We mark clearly which print titles come from scanned image books so that you can make an informed purchase decision about the quality of what you will receive.

Original electronic format
These ebooks were created from the original electronic layout files, and therefore are fully text searchable. Also, their file size tends to be smaller than scanned image books. Most newer books are in the original electronic format. Both download and print editions of such books should be high quality.
